We are currently recruiting for the position of  Nurse Flying, based in Jowhar.

General Purpose of the Job:

Flexibility is required to work in different departments of the project. The responsibility requires working inOPD,SCand Maternity (only Jowhar project) according to MSF protocol and MSF standards. The position is flying, so you are required to cover consultant during their holydays or other absences.

General tasks are to ensure nursing care, treatment and follow-up of patients and/or a population in accordance with doctors’ prescriptions, MSF protocols, respect of universal hygiene standards and context. Ensure supervision and training of a team.

Specific Responsibilities and Tasks:

  1. 1.    Patient care
  • Know, apply and make sure universal hygiene standards and security rules are respected for medical premises, routing of patients, routing of linen. Ensure asepsis rules during all nursing cares. Ensure quality of the sterilization process.
  • Compensate (or make sure that someone can compensate in the right way) patient lack of autonomy especially regarding his feeding, his personal hygiene, his movements and his comfort.
  • Organise and carry out care and treatments according to doctor’s prescription and MSF protocols.
  • Ensure that quality and continuity of care are respected.
  • Carry out surveillance of the patient, assessment of his health state evolution, identification of emergency situation and information of the doctor.
  • In case of emergency, carry out first aid care and treatment according to prior written protocol done by the doctor.
  • During medical consultations, ensure the sorting and detect priority acute cases.

Carry out when necessary a therapeutic education of the patient (and family) using a patient based approach.

  1. 2.    Management
  • Manage daily and motivate a team of nurse-helpers and cleaners. Supervise the quality of care. Ensure staff training (formal and informal), under the supervision of the hierarchical referent.
  • Manage efficiently the pharmacy and medical equipment: drugs orders, follow-up of the stock, storage conditions, inventories, follow-up of expired drugs and their destruction
  1. 3.       Monitoring, analysis and reporting
  • Participate in the development of the plan of action
  • Participate actively in the monitoring and the reporting of medical activities (correctly fill in patient files, forms, statistics, reports) and propose analyse of these statistics.
  • Ensure a relevant transfer of information to the next duty team.
  • Report immediately to the technical referent any problem arising in the service, especially the loss/ robbery/damage of medical equipment or medicines
  1. 4.    When working in the Admission Room
  2. 5.    General on admission in all wards
  • During the afternoon and night shifts take a proper history and do a physical examination of the patients presenting in the admission room.
  • Assess the condition of the patients and fill in the patient card accordingly.
  • Decide if treatment can be given, and if the patient should be sent home or has to be admitted.
  • Evaluate if there is a need to call the clinical officer.
  • Assist the CO/MD with emergency procedures. Provide first aid if necessary.
  • Fill in correctly the register book.
  • Provide nursing care according to the prescription.
  • Inform the patients and caretakers about the routines in the health centre, the locations and use of water sources, use of latrines, feeding times etc.
  • Assess the condition of the patient, and record you findings in the patient file.
  • Record in the register book(s) the required information.
  • Ensure that each patient receive mattress, mosquito net and soap.
  • Ensure the patient is vaccinated in OPD (if is a paediatric patient): if not, follow up and make sure this is done. Document in the patient file, and educate the patient/caretaker about EPI.
  • Give information to the patient and the caretaker about their rights and responsibilities while admitted and about the care and the treatment and its side-effects.
  • Inform the patient/caretaker about the routines in the health centre, the locations and use of the water sources, use of latrines, feeding times etc
  1. 6.    General nursing care in all wards
  • Provide nursing care according to the prescription.
  • Control vital signs such as Blood Pressure, Temperature, Pulse and Respiratory rate. Analyse the results and respond accordingly.
  • Assess, follow up and evaluate the condition of all admitted patients, and inform the Supervisor/Clinical Officer or Medical Doctor if any problem or deterioration in the patient’s condition.
  • Put a nasogastric tube, urinary catheter or IV cannula if prescribed by MD/CO.
  • Follow up closely when use of IV line: give medications accordingly to prescription, and record the time and the quantities given when using IV fluids.
  • In collaboration with the nutritionists follow up the patients in the ITFC, weight gain, appetite, observations of oedema, anaemia, dehydration etc.
  • Assist the medical doctor/CO during the in-patient rounds.
  • Assist the medical doctor/CO when interviewing and examining patients when needed.
  • Ensure that the in-patients receive food.
  • Give general health education to patient/caretakers on admission and every day during their stay in the Health Centre.
  • Inform the patients/caretakers about the treatment and  medication given, and ensure that the message is understood.
  • Ensure that the patient gets the medication, and if vomiting or loss that the correct new amount is given.
  • Follow up patients that are not present during the ward or medication rounds.
  • Collect drugs in the pharmacy, if necessary.
  • Prepare the trolley for the medical round.
  • Distribute the medications prescribed and confirm the following; right patient, right time, right dosage, right drug and check the expiry date of the drug.
  • Participate in planning and organizing the work.
  • Apply dressing according to the prescription and to the wound classification treatment protocol. Adapt the wounds treatment according to the healing stages.
  • Inform patients/caretakers about the importance of finalizing initiated treatment, if there is a need to leave the HC to wait for the CO/MD to prescribe treatment for use at home.
  • Encourage the mother to stimulate her child. Play is important for children growing.
  • In close collaboration with the other nurses and nutritionists, ensure that the ward is never left unattended.
  1. 7.    General documentation in all wards
  • Write observations down, and make note if changes in patient’s condition.
  • Documentation in medical documents of patient’s condition.
  • Documentation in medical treatment cards of treatment given.
  • Report daily activities.
  • Report admissions, defaulters and deaths.
  • Written and oral handover  report to colleagues about patient’s condition and prescribed treatment.
  • Ensure, together with supervisor,  that the register books (both ITFC and ward) are updated daily.
  • Fill referral forms when requested.
  1. 8.    General administration/ organization in the  wards
  • Together with the supervisor ensure the white board are updated with numbers of patients, and that the information needed for correct food supply is updated.
  • Follow the responsibility for the day as directed by supervisor. Help the others when you are finished with your duties.
  • Beginning and end of each shift, find out how many beds you have free to make ready for new admission
  • If requested by the MD/CO ensure that lab tests are performed and result is documented in the patients file.
  • Ensure hygiene in pharmacy management and drug handling .
  • Prepare disinfecting solution when needed.
  • Responsible for ensuring proper hygiene conditions in the ward (good ventilation, incoming sunlight, etc.). Educate patients and caretakers to use the latrines, potties, etc.
  • You must ensure you dispose of waste according to the system in place in each ward: correct use of sharps container, buckets etc.
  1. 9.    General on discharge:
  • Ensure that the patient are well informed about his disease, what treatment he has been given and ,if possible, how the disease can be prevented in the future.
  • Document the patient as discharged from the register book and remove the patient card and file it.
  • Fill out and give the patient/caretaker referral letter if requested by MD/CO
  • If necessary inform that the patient must come back for EPI.
  • Specific for KA: ensure that the patient card is completely filled in, inform about the risk of relapse and importance if coming back in case of relapse. Follow the patient to OPD for vaccinations and discharge food.

10.Specific for working in TB ward:

  • Assist in the DOTS clinic when needed.
  • Perform the DOTS pharmacy activities on Fridays according to rotational schedule, and when the assigned person is on leave.
  • Participate in the regular health education sessions for all TB patients

Additional Tasks:

  • Be flexible and be able to give assistance with different activities not included in this primary job description.

 

Requirements:

Nationality:

  • Somali

Education:

  • Nurse degree/diploma recognised by National Authorities.

Experience:

  • None is required MSF experience and other.

Language:

  • Fluent spoken & written English
  • Fluent spoken & written Somali (a must)
